Register by mail following these steps:
Step 1: Click on these 3 forms to download and print them.
Step 2: Fill out each form completely.
Step 3: Mail all three forms to the following address:

PLEASE BE SURE AND INCLUDE:
1. Copy of child’s birth certificate or passport registration. (Required)
2. Check or money order for the registration fees listed below:
* U-6 & U-8 ..........$55
* U-10 & U-12.......$65
* U-13 & up ..........$75
To determine the total amount due for the child or children you are registering. If you are registering more than one child, deduct $10 from the required fee for each additional child. Additional children must be siblings, or you must be the legal guardian.
** Must have all three forms for each child being registered **
